We will be discussing any questions or concerns you may have regarding this document on your first visit. Please read it thoroughly and jot down any concerns or inquiries.
How your health information will be used and the rights and limits of confidentiality is also very important, and I encourage you to please print, read and complete the privacy policy forms.
The client information sheet is also available and will beneeded for your first visit. If you are unable to print these forms, please arrive 20 minutes before your first visit to allow time to complete these documents.
